Application Process

 

FIRST, ensure that you meet the Eligibility Requirements. 

 

NEXT, complete the application steps 1, 2 and 3.

 

Step 1: Complete and submit by 5pm September 6th, 2019 to your Peer Mentor:

Submit the PD Stipend Application either by:

or mail:

Step 2: Create a PD Plan with your peer mentor and complete activities in your PD Plan. Submit by May 15th, 2020. 

Step 3:  Complete and submit to your Peer Mentor by 5pm May 15th, 2020: 

  • PD Stipend Request Form

  • Mandatory survey

  • Verification of Completed Activities and/or Documentation, as indicated in your plan

Please Note:

 

Email is the primary communication for CCCC. Therefore, participants must have access to a valid email account.

 

Stipends must be declared as income and are subject to income tax regulations. A 1099 form will be mailed to any participant who received total payments of more than $600 during the 2018-2019 year.

Stipend Amount

Stipend amounts vary depending on professional development options completed, funding, and the number of successful applicants. 

Moving?

Stipends are processed mid-July of each year and will be mailed to the address listed on the W-9 on file. If you move during the year, please fill out a new W-9 form and notify us by sending an email to rdort@nevco.org or lstewart@nevco.org.
